The concept of hysteria is traced from Hippocrates, where it was thought to be caused by a wandering uterus, through Galen and up to Freud. Throughout the history of medicine from the early Greeks up to the end of the nineteenth century, the definition and diagnosis of hysteria had a function similar to that found in the persecution of witchcraft: it sought to eradicate the outbursts of nonconforming and emotionally threatening conduct of women. The current U.S. health care system, with both rising costs and demands, is unsustainable. The combination of a sense of individual entitlement to health care and limited acceptance of individual responsibility with respect to personal health has contributed to a system which overspends and underperforms. This sense of entitlement has its roots in a perceived right to health care. Beginning with the so-called moral right to health care, the issue of who provides health care has evolved as individual rights have (...) trumped societal rights. The concept of government providing some level of health care ranges from limited government intervention, a ‘negative right to health care’, to various forms of a ‘positive right to health care’. The latter ranges from a decent minimum level of care to the best possible health care with access for all. We clarify the concept of legal rights as an entitlement to health care and present distributive and social justice counter arguments to present health care as a privilege that can be provided/earned/altered/revoked by governments. We propose that unlike a ‘right’, which is unconditional, a ‘privilege’ has limitations. Going forward, expectations about what will be made available should be lowered while taking personal responsibility for one’s health must for elevated. To have access to health care in the future will mean some loss of personal rights and an increase in personal responsibility for gaining or maintaining one’s health. (shrink)

Mood disorders, including major depressive disorder and bipolar disorder, are highly prevalent, frequently disabling, and sometimes deadly. Additional research and more effective medications are desperately needed, but clinical trials research in mood disorders is fraught with ethical issues. Although many authors have discussed these issues, most do so from a theoretical viewpoint. This manuscript uses available empirical data to inform a discussion of the primary ethical issues raised in mood disorders research. These include issues of consent and decision-making capacity, including (...) patients’ motivations for participating in research. We also address drug withdrawals, placebo controls, and the overall safety of research. Finally, we examine the extant literature for studies discussing potential indirect benefits of clinical trials research to participants. Taken together, the evidence suggests that clinical trials research incorporating drug withdrawals and placebo controls can be conducted safely and ethically, even in patients with severe or treatment-resistant mood disorders. In fact, given the dearth of effective treatment options for this population, it is our opinion that a moral imperative exists to extend the offer of research participation to severely ill or treatment-resistant groups. Although ethics is an essential component of undergraduate medical education, research suggests current medical ethics curricula face considerable challenges in improving students’ ethical reasoning. This paper discusses these challenges and introduces a promising new mode of graduate and professional ethics instruction for overcoming them. We begin by describing common ethics curricula, focusing in particular on established problems with current approaches. Next, we describe a novel method of ethics education and assessment for medical students that we have devised, the Medical Ethics (...) Bowl. This article seeks to develop theoretical convergences between the science of nostalgia and the science of heroism. We take four approaches in forging a conceptual relationship between these two phenomena. First, we examine the definitions of nostalgia and heroism from scholars, laypeople, and across cultures, noting how the history of defining the two phenomena has shaped current conceptualizations. Second, we demonstrate how nostalgic experiences consist of reminiscences about our own personal heroism and about cultural role models and heroes. A review (...) of heroism research, moreover, shows also that our recall of our heroes and of heroism is tinged with nostalgia. Third, we make linkages between heroism and nostalgia research focusing on functions, inspiration, sociality, and motivation. Nostalgia researchers have illuminated the functions of nostalgia implicating the self, existential concerns, goal pursuit, and sociality. Our review shows that heroism researchers invoke similar categories of hero functionality. Finally, we propose three areas of future research that can profit from the merging of nostalgia and heroism science, involving the mechanisms by which heroism can fuel nostalgia, nostalgia can promote heroic action, and wisdom results from nostalgic reverie. Although Christ’s atoning work on the cross is perhaps the most central tenet of Christianity, understanding precisely how the cross saves remains a theological mystery. We follow the Abelardian tradition and argue that Christ’s death on the cross acts as an example of God’s love for humanity and a means of drawing us back into communion with the triune God. However, our view avoids the standard objection to exemplar views—that they are Pelagian—by introducing an alternative conception of the problem of (...) sin, according to which Christ’s example of God’s love is in fact required for salvation and sanctification. This study examined within-person co-occurrence of self-focus, negative affect, and stress in a community sample of adolescents with or without emotional disorders. As part of a larger study, 278 adolescents were interviewed about emotional disorders. Later, they completed diary measures over three days, six times a day, reporting their current thoughts, affect, and levels of stress. Negative affect was independently related to both concurrent stress and self-focus. Importantly, the association between negative affect and self-focus was stronger among participants with a (...) recent unipolar mood disorder, compared to those with an anxiety disorder, comorbid anxiety and depression, or those without an emotional disorder. The implications of these findings to theories of self-focus and its role in emotional disorders are discussed. (shrink)

Background Community advisory boards have expanded beyond high-income countries and play an increasing role in low- and middle-income country research. Much research has examined CABs in HICs, but less is known about CABs in LMICs. The purposes of this scoping review are to examine the creation and implementation of CABs in LMICs, including identifying frequently reported challenges, and to discuss implications for research ethics. Methods We searched five databases for publications describing or evaluating CABs in LMICs. Two researchers independently reviewed (...) articles for inclusion. Data related to the following aspects of CABs were extracted from included publications: time, country, financial support, research focus, responsibilities, and challenges. Thematic analyses were used to summarize textual data describing challenges. Results Our search yielded 2005 citations, 83 of which were deemed eligible for inclusion. Most studies were published between 2010 and 2017. Upper-middle-income countries were more likely to have studies describing CABs, with South Africa, China, and Thailand having the greatest numbers. The United States National Institutes of Health was the main source of financial support for CABs. Many CABs focused on HIV research. Thirty-four studies reported how CABs influenced the informed consent process for clinical trials or other aspects of research ethics. CAB responsibilities were related to clinical trials, including reviewing study protocols, educating local communities about research activities, and promoting the ethical conduct of research. Challenges faced by CABs included the following: incomplete ethical regulations and guidance; limited knowledge of science among members of communities and CABs; unstable and unbalanced power relationships between researchers and local communities; poor CAB management, including lack of formal participation structures and absence of CAB leadership; competing demands for time that limited participation in CAB activities; and language barriers between research staff and community members. Several challenges reflected shortcomings within the research team. Conclusions Our findings examine the formation and implementation of CABs in LMICs and identify several ethical challenges. These findings suggest the need for further ethics training among CAB members and researchers in LMICs. (shrink)

This article describes some potential uses of Bayesian estimation for time-series and panel data models by incorporating information from prior probabilities in addition to observed data. Drawing on econometrics and other literatures we illustrate the use of informative “shrinkage” or “small variance” priors while extending prior work on the general cross-lagged panel model. Using a panel dataset of national income and subjective well-being we describe three key benefits of these priors. First, they shrink parameter estimates toward zero or toward each (...) other for time-varying parameters, which lends additional support for an income → SWB effect that is not supported with maximum likelihood. This is useful because, second, these priors increase model parsimony and the stability of estimates and thus improve out-of-sample predictions and interpretability, which means estimated effect should also be more trustworthy than under ML. Third, these priors allow estimating otherwise under-identified models under ML, allowing higher-order lagged effects and time-varying parameters that are otherwise impossible to estimate using observed data alone. In conclusion we note some of the responsibilities that come with the use of priors which, departing from typical commentaries on their scientific applications, we describe as involving reflection on how best to apply modeling tools to address matters of worldly concern. Si le déploiement d’une compétence éthique en situation nécessite la capacité et la volonté de la personne à déceler et à résoudre les problèmes de nature éthique qu’il rencontre, je proposerai dans ce texte qu’il nécessite aussi, en raison de la part d’autonomie qu’il requiert, que la personne ait le pouvoir de déployer cette compétence. Or, alors que les deux premières conditions d’exercice dépendent de la personne, cette troisième condition, à laquelle je m’intéresserai plus particulièrement dans ce texte, lui est (...) pour sa part externe. Ainsi, je proposerai que c’est non seulement la personne, mais aussi un enchevêtrement d’éléments du contexte, de la situation et surtout, du réseau d’interactions dans lesquels une personne s’insère qui, parce qu’ils exercent sur elle des pouvoirs, détermineront si la personne aura la possibilité d’agir de façon compétente en situation éthique. Ainsi, s’intéresser à la compétence éthique impliquera nécessairement de s’intéresser à la question du pouvoir et à la façon dont s’exerceront les pouvoirs en situation – des pouvoirs sur ou des pouvoirs avec – puisque cela aura une influence déterminante sur le pouvoir de la personne de déployer sa compétence de façon effective en situation. La visualisation textuelle est une technique qui utilise des graphiques et des diagrammes afin d’examiner des textes sous la forme de données. Ces graphiques et diagrammes, en général, ne représentent pas les textes de manière directe, mais présentent des résultats fondés sur le nombre de mots, sur les termes en séquence, et ainsi de suite. Cette technique peut révéler des mots-clés importants, donner un aperçu du contenu textuel, faire émerger des tendances récurrentes dans un seul texte ou à travers un (...) corpus. Cet article décrit les avancées récentes de ce champ de recherche et les expériences menées avec ces nouveaux outils pour le Grub Street Project, lesquelles consistent à générer des visualisations de textes du dix-huitième siècle, en portant notamment une attention particulière aux rapports spatiaux entre les mots et les entités dans les documents originaux. Although we find Gangestad & Simpson's argument intriguing, we question some of its underlying assumptions, including: (1) that fluctuating asymmetry (FA) is consistently heritable; (2) that symmetry is driving the effects; (3) that use of parametric tests with FA is appropriate; and (4) that a short-term mating strategy produces more offspring than a long-term strategy.

The first two sections of this paper are devoted respectively to the criticisms of my views raised by Stephen Engstrom and Andrews Reath at a symposium on Kant's Theory of Freedom held in Washington D.C. on 28 December 1992 under the auspices of the North American Kant Society. The third section contains my response to the remarks of Marcia Baron at a second symposium in Chicago on 24 April 1993 at the APA Western Division meetings. The fourth section deals with (...) some general criticisms of my treatment of Kant's theory of freedom and its connection with transcendental idealism that have been raised by Karl Ameriks, who was also a participant in the second symposium, in an earlier piece published in Inquiry and by Paul Guyer in a review. Este artículo problematiza la forma en la que el feminismo ha interpretado al escepticismo como una amenaza para su proyecto epistemológico, ético y político, y plantea que, a pesar de dicha interpretación, el escepticismo es inherente a la filosofía feminista y un aliado útil para su proyecto. El problema ha estado en cómo el feminismo ha reducido su comprensión del escepticismo a una versión cartesiana extrema y ha pasado por alto otras corrientes de la tradición —como la de David Hume— (...) que podrían ser fructíferas para promover una actitud crítica y emancipatoria. Muestra así como una alianza entre una comprensión moderada del escepticismo y el feminismo puede brindar las herramientas necesarias para cuestionar formas de dominación patriarcal y poner en marcha investigaciones que combatan la opresión de género. Edited by the author of The Lonely Labyrinth, this anthology is a superb collection of Kierkegaard studies. It begins with two general statements of Kierkegaard’s thought: Louis Mackey’s previously published and tightly packed essay "The Poetry of Inwardness," and a chapter from Prof. Thompson’s newest book on Kierkegaard entitled "The Master of Irony." There is also Sartre’s essay "The Singular Universal" from Kierkegaard Vivant and an interesting historical essay by Richard Popkin which situates Kierkegaard in the history of modern skepticism (...) and shows his affinities with Hume. If there is a common theme to this collection it is the notion that Kierkegaard has persistently separated himself from the statements of his pseudonymous authors and that what has come to be known as "Kierkegaard’s philosophy" is really the product of Kierkegaard’s irony. Otherwise Kierkegaard’s entire literary effort becomes self-contradictory. The essays by Thompson, Crites and Allison are especially relevant in this regard. Editor Thompson has also done us the splendid service of updating the 1962 Kierkegaard Bibliografi with a supplement which includes all the literature in English for the years 1956-70. There is also a most helpful index locurum of the works of Kierkegaard cited in this anthology.—J.D.C. (shrink)
